Report an accident

The accident/incident report form should be used to report all accidents, incidents and near misses which involve WDC employees whilst they are at work, or which involve non-WDC employees affected by WDC premises or activities.

Please take every care to complete the form carefully and legibly, this document can be used as evidence in court. Failure to meet these requirements could result in:

  • an investigation not identifying the true causes of an accident or incident, therefore rendering its findings unsafe.  
  • delays in corrective action being taken.

If the incident relates to a non-employee then a member of WDC staff should normally complete the form on their behalf.
